(Note: This is a Non-Prescription Drug and Alcohol free event)

Registration:  You can register in one of two ways:

1) Simply click on the PDF form buttons above for the event Schedule and Registration and let the documents open on your computer.

A. Click the Schedule & Details form for event information that can be printed.

B. Click the ONLINE form which can be filled out on your computer then printed.

2) By doing the above you can download the PDF file above and open it.  Once downloaded, Open the form.  You can then, print the blank form or fill it out on your computer then print it. 

Each information line on the form is designed to be typed into and then printed out.

Be sure to Sign and Date the form, and send back to us with the payment.

3) Return the Registration form with a check for $15.00  for each Participant to FHNB 52 East Crescentville Road, Suite E  West Chester Ohio 45246
